During the events of the add-on Broken Steel , if the player had chosen to contaminate the purifier with the modified FEV, what President Eden said is confirmed for as accurate and the FEV completely lethal.
These new patients hold their stomachs and repeat dialogue along the lines of, "it burns on the inside. This occurs in both humans and ghouls , despite the latter's immunity to most diseases. The FEV is contained to only those that have ingested infected water as the transmission of FEV is impossible from physical contact or bodily fluids. In Rivet City , Dr. Preston comments that they are burning the corpses just to be safe. However, eventually, the spread of the FEV infected water will spread from the Tidal Basin and affect all water in the Capital Wasteland.

In the case of using the FEV virus, it will kill all remaining life. There is also the fact that the game contains DLC that would be rendered useless based on this decision. It WILL do something, but you will not get to witness it. Spoiler time! When you use the virus, you will get a nice cutscene to show how the virus kills everyone and wipes out rivet city. Fairly pointless really. On the other hand, by not using it you are an all-round good guy or gal, and now have access to the aqua pure water that is a handy addition to keep you alive wasteland.

Ok I've done some asking around the towns aka clicking on random settlers , and a few people have made reference to a "sickness" that's spreading around so I can now see what you guys meant about "side effects" One guy in megaton told me it'd be best if I left because of it.
I also noticed the clinic was full of sick, unconscience people as well so I guess that's my questions answered. Thank you to anyone who tried to give a civilized, helpful response to this topic. After I blacked out, there was the ending cut scene showing a lot of dead mutants and anyone infected by radiation etc all over the place, even at Rivot city, and gave some claim that the wastes has become "a grave yard", but after the game resumes I went there to check and NO-ONE is DEAD!!
Also there are still Super Mutants running around at their camp just down the road! How do you know who died? Why does it claim that all mutated people etc were killed off by entering the virus into the water if they weren't?
Whats up with that? I hate liars!
